I understand and agree, that a background check may be ran on myself, and the abuser that is accused.  I further understand that my situation will be verified for the safety of the non-profit, and the safety of those I may receive help from.   I attest that all information given is truthful to best of my knowledge and that i can be held accountable for any information given to the Operator Project deemed false.
